I am an anesthesiologist who practices neuro-anesthesia and I am the Medical Director of the Ohio State Wexner Medical Center Preoperative Assessment Center, (OPAC). I have been in this position since 2011, prior to that I was a Co-director from 2009-2011. The OSU Preoperative Assessment Center (OPAC) sees medically complex patients prior to surgery. These patients are classified as American Society of Anesthesiologists physical status 3 (a patient with severe systemic disease) and 4 (a patient with severe systemic disease that is a constant threat to life). The clinic sees patients from multiple surgical specialties; many come from out of state or out of the country.
I approach leadership of the OPAC with efficiency and lean management strategies. I have participated in Ohio State’s Faculty Advancement, Mentoring and Engagement (FAME) program. I have also so taken the Fisher College of Business’s Lean Six Sigma online course, and I have graduated from Ohio State's Fisher College of Business with a Master of Business Operational Excellence degree. I actively add lean measures in the clinic to improve our patient flow and ultimately allow us to increase our volume. This boosts personalized care, patient safety and decreases same day surgery cancellations.
I also apply lean and operational efficiency strategies to the OPAC role by troubleshooting systemic problems. I identify areas for improvement and implement changes in how we assess our patients. This information is conveyed to the surgeon, anesthesia team and the OR team. Detailed analysis of potential problems and pitfalls, and increasing communication about these issues, has significantly decreased day of surgery cancellations. OPAC patients have less same day cancellations,less than 3 percent, compared to the rest of the surgical population which is greater than 10 percent. This also results in significant cost-savings, as an hour of operating room time is approximately $2000.00, (not factoring in lost supply costs and idle staff).
An essential aspect of my leadership in managing this complex surgical patient population is to conducting root-cause analyses for any OPAC patient where an issue was encountered or surgery was cancelled. In this way, we circumvent same and similar issues happening in the future, proactively improving patient care and outcome. This also maximizes the OPAC’s operational efficiency.
In July 2013 the Department of Anesthesiology and the Department of Internal Medicine partnered to improve the OPAC experience. To capitalize upon this interprofessional collaboration, as well as to increase our capacity to see patients, I advocated a new staffing model and implemented it in 2015. This model calls for a mix of midlevel providers and several internal medicine physicians, along with an anesthesiologist. They key component of this new model is the recruitment of three internal medicine physicians with non-overlapping backgrounds. This gives the clinic a breadth of knowledge to assist in the care and evaluation of our complex patients. It has allowed us to simultaneously facilitate both internal medicine and anesthesia preoperative consults, as well as all same day preoperative assessment requests. This has improved our efficiency and communication because everyone is physically in the same office. This is a distinctively different preoperative assessment center model than any other in central Ohio. I have received a request for visitation and ideas on how to open this type of center from our largest competitor, Ohio Health.
I educate residents and medical students, as well as lecture on preoperative medicine. I am involved in preoperative medicine research and I am the primary investigator on two research projects that involve preoperative patients .One study is published and the other is in review. I am also a co-investigator on multiple other studies. I have been invited to be on several national committees, where I am able to share my preoperative experience and expertise.
